Doing well together, episode 2: Optimizing staff resources and reducing costs with digital companions

A majority of patient’s health is impacted by factors outside a traditional healthcare visit. As a provider organization, how can you continue to impact a patient’s health in the space between visits?
 
Many organizations are incorporating digital companion programs to positively influence patient health before, between and after visits, while simultaneously optimizing staff resources and lowering costs.

Listen in as Erica Auger, Senior Director, Ambulatory Care Management at Corewell Health, discusses how Corewell Health has incorporated digital companion programs into its existing workflows, including its ED Discharge program that has been a significant time and cost saver for the system. In just one year, the program drove a 5% reduction in ED readmissions, and a reduction in the number of staff needed to manage follow ups.
